School Medication Policy
- All medications (prescription and over-the-counter) require physician authorization and parent/guardian consent.
- Medication forms must be renewed annually.
- Medications must be in original containers, labeled with the student's name.
- Parents/guardians must deliver medications to the school nurse.
- Students may not carry medications, except for inhalers and Epi-Pens with proper authorization.
- Asthma inhalers and Epi-Pens require physician's note confirming student's ability to self-administer.
- Unused medications should be collected by parents/guardians at year-end.
- For severe allergies, parents must discuss Epi-Pen designees with the school nurse.

School Nurse
The Washington Borough School District, in compliance with the requirements of the New Jersey Department of Education and the local Board of Education, mandates that the following forms must be completed by the beginning of the school year for your child to enroll in district schools:
- Health History form to be completed by a parent or guardian
- A recent Physical Examination form completed by a physician
- An Immunization Schedule from your physician with up-to-date immunizations
- A Medical Concerns form (if your child has additional medical issues, such as diabetes, allergies, etc.)
Additionally, effective September 2016, students entering 6th grade must provide documentation of having received the following vaccines:
- Tdap Vaccine (Boostrix, Adacel): Students born after January 1, 1997, and enrolled in Washington Borough School District from another state or country must receive one dose of Tdap (tetanus, diphtheria, acellular pertussis) given no earlier than their 10th birthday, as per N.J.A.C.8:57-4.10.
- Meningococcal Vaccine (Menactra, Mennomune): Students born after January 1, 1997, and enrolled in grade 6 or transferring into Washington Borough School District from another state or country must receive one dose of meningococcal (meningitis) vaccine, according to N.J.A.C.8:57-4.20.
