Vendor Forms

The following forms must be completed in order to become a new vendor for the district. Click the form name to access the forms. Once you have completed the forms, please send all documentation and/or any questions you may have to howleyc@washboro.org with the words “NEW VENDOR FORMS” in the subject line.

VENDOR REGISTRATION FORM – required from every vendor doing business with Washington Borough School District

W-9 FORM – required from every vendor doing business with Washington Borough School District

NJ BUSINESS REGISTRATION CERTIFICATE – required for vendors doing $6,600 or more in business per fiscal year with Washington Borough School District

POLITICAL CONTRIBUTION DISCLOSURE FORM C.271 (PAY TO PLAY) – required for vendors doing $17,500 or more in business per fiscal year with Washington Borough School District

AFFIRMATIVE ACTION FORM AA302 – required for construction vendors and certain other vendors doing $44,000 or more in business per fiscal year with Washington Borough School District

PROHIBITED ACTIVITIES IN RUSSIA & BELARUS AND INVESTMENT ACTIVITIES IN IRAN– required for vendors doing $6,600 or more in business per fiscal year with Washington Borough School District School District

You will receive a confirmation email once you have successfully been added as a new vendor or if additional information is required.
